Pediatric Sick/Urgent Care Visit:
$100 per first patient
$50 per each additional patient being seen at the same visit
If a follow-up visit is needed within 48 hours after a home visit: $50
Telehealth Sick Visit
$50 per patient
(Pediatric Telehealth sick visits when applicable based on provider discretion)
School Sports Physicals
$75 per patient
(Excludes immunizations)

REMINDER
Bloom does not accept and/or bill insurance, allowing our practitioners to remain transparent, provide personalized care, and and focus entirely on your family’s wellness. Accepted forms of payment include Zelle, Venmo, cash or check. Bloom’s practitioners will always review costs with you before services are provided, and we are happy to provide receipts you may use for reimbursement (Bloom is not involved in the submission or insurance reimbursement process.) Pricing and an approximate total will be provided to the patient/parent before the requested services begin. Add-ons may increase the total due upon completion.
